What you share

The appointment form asks for your name or initials, a contact method and address or number, your chosen service and optional preferred time. Please do not submit medical history or documents.

How it is used

With your agreement, your request is sent to the clinic by email so the team can reply and arrange a visit. This website does not create a patient account or store a copy of your request in a website database. Copies remain with the email providers and in the clinic’s mailbox.

Services involved

Cloudflare hosts the site and handles form requests and Turnstile spam protection. Resend delivers the request email. These services process information needed to provide their functions, potentially outside Malaysia. Google processes requests when you load the embedded map or the website’s fonts. WhatsApp handles messages when you choose that contact method.

Your choices

You can call instead of submitting a form. The HIV testing guide works in your browser and does not send or save your selection. We do not add advertising or visitor analytics trackers to this website.

Contact [email protected] about access, correction, deletion or how the clinic retains correspondence. Clinical record requirements are handled separately by the clinic.
